This book describes the most recent insights into heart failure and the role played by autonomic nervous system pathophysiology in it, discussing the therapeutic implications. While current therapeutic approaches are able to control the effects of excessive adrenergic activation in heart failure syndrome, the underlying abnormalities of adrenergic control remain unaltered and can still cause progression to unmanageable end-stage heart failure. New therapeutic pathways are therefore being explored with a view to developing interventions that can directly modulate adrenergic over-activity and restore a more appropriate balance in neural control of the cardiovascular system. The book opens by examining current heart failure therapies. Advances in our understanding of autonomic regulation/dysregulation in heart failure are then discussed in detail, in the context of the search for more effective therapies. A concluding section addresses the role of autonomic nervous system denervation in heart failure. The authors are top scientists from leading research centers.

In the last years, indications for defibrillators and cardiac resynchronization therapy have expanded enormously; for this reason, and also due to the extension of human life length, the number of patients with implanted cardiac devices have steadily increased. The leads implanted for the functioning of these devices, however, have a limited duration in time and more and more their extraction will be a frequent issue in clinical practice, in order to treat short- and long-term complications, such as infections and failures.Aim of this book is to provide readers with a state-of-the-art on lead extraction techniques. The chapters deal with leads characteristics, indications to lead removal, patient preparation, tools and techniques for extraction, and prevention and management of complications. Wir freuen uns sehr dariiber, daB das Mlinchner Blutdruck-Programm (MBP), welches Anfang der 80er Jahre mit relativ geringen finanziel­ len Mitteln in Mlinchen begonnen wurde, nieht nur erfolgreich abge­ schloss en werden konnte, sondern nun mit der Verleihung des "Hufe­ land-Preises 1989 fur vorbeugende Gesundheitspflege" eine so hohe Auszeichnung erfahren hat. Schon zu Beginn des Mlinchner Blutdruck-Programmes haben wir immer wieder betont, daB wir den Risikofaktor Hypertonie als beson­ ders gut anzugehenden Einstiegsrisiko/aktoransehen und daB wir den beiden anderen klassischen Risikofaktoren Zigarettenrauchen und Hypercholesterinamie ebenso groBe Bedeutung fur die Verursachung von Herzkreislaufkrankheiten beimessen. Es ist tatsachlich so, daB aus den groBen epidemiologischen Kohortenstudien der letzten Jahr­ zehnte eindeutig hervorgeht, daB die Risikofaktoren Hypertonie, Hypercholesterinamie und Zigarettenrauchen mit ahnlich hohen Risi­ ken fur Herzkreislaufkrankheiten einhergehen und bei Zusammen­ treffen eine synergetische Wirkung entfalten. The aim of present-day treatment of hypertension is no longer as the effective reduction of blood pressure, regarded merely but as the ever-increasing application of new therapeutic prin­ ciples to prevent such consequences of hypertension as left heart hypertrophy, medial hypertrophy of the vessels and ar­ teriosclerosis. In this context special attention has been di­ rected to stimulation of the endogenous vasocilator hormonal systems such as the prostaglandins. The great interest in our book, which has meanwhile appeared in a French edition, seems to confirm this trend and has encouraged us to include in the book the new findings published in the last twelve months. These changes and additions maintain the current status of the book and provide an even more complete survey of the available information about prostacyclin in hyperten­ sion. Hypertension, as evidenced by a pathologic increase in blood pressure, is one of the most important risk factors. Primary hypertension can be established in about 15 % of adults. Patients with hyperten­ sion usually feel well subjectively and only rarely complain of symptoms.

Some of the leading scientists in the field of hypertension accepted an invi­ tation to actively participate in an international symposium on primary hypertension. Scientists from 13 countries - from western, southern, and eastern Europe, Australia, and the United States - were present. The meeting was held in Cologne in March 1985. After previous meetings and workshops at Titisee in the Black Forest and several symposia on the renin­ angiotensin-aldosterone system here in Cologne, this symposium on prima­ ry hypertension was organized in order to induce and to stimulate critical discussion on the basic pathogenetic mechanisms involved and the recently established therapeutic implications. Numerous studies have been based on the concept that the elevation in blood pressure is mediated by several vasopressor substances. For this reason the renin-angiotensin system has been considered to be the essential endocrine mechanism involved in developing and sustaining arterial hyper­ tension. Other vasopressor factors, such as catecholamines, vasopressin, and serotonin, have been studied extensively and were assumed to play additional parts in blood pressure regulation. However, several other ap­ proaches have been initiated in recent years.
